TRAINING APPLICATION / WAVIER FORM

All students must fill out this form completely and bring it with
them on the first day of training.  This is a requirement to
participate in any part of our training weekend.  No exceptions.
To pay with a check or money order:

1.  Fill out online form above (STEP 1)
2.  Download and print Application / Waiver Form (STEP 2)
3.  Make check or money order out to: Northern Virginia Fools
4.  Bring your check / MO along with your COMPLETED Application / Waiver Forms to the
training site prior to the start of class.  Do not mail waivers or payments.
